  3. Attach a copy of Rabies Certificate and spayed/neutered proof with this application.
  4. Mail or bring it to the Health Department or put it in the drop box by the front door of the Municipal Building. Do not forget to include a check or money order for the correct amount made out to Rockaway Township. If you have any questions about how much the license will cost, please call the Health Department at 973-983-2848/2849 and someone will assist you.

  Attach a copy of Rabies Certificate and spayed/neutered proof with this application.
